Adjustment & Repair Waiver

• •This waiver applies to frames and lenses not originally purchased from AUHSY Optical.
• •Our team will handle your eyewear with the utmost care and do our best to adjust or repair your frame.
• •Before any work begins, an AUHSY Optical team member will inspect your frame and lenses for any visible damage. If any is found, you will be informed before the service continues.
• •Please be aware that frames and lenses may have existing hairline cracks or damage that are not visible to the naked eye.
• •If, during the adjustment or repair process, a frame with such pre-existing (undetected) damage is damaged or breaks, AUHSY Optical is not responsible.
• •By signing below, you acknowledge that you have read and understood this waiver and release AUHSY Optical from liability for damage resulting from pre-existing, undetected damage during adjustment or repair of patient-owned frames.
